The Mazda Daily Rental Fleet Program is intended to encourage the use of Mazda products by the Daily Rental Fleet Companies.
PROGRAM PERIOD:
The 2009 model year Mazda Daily Rental Fleet Program is effective July 1, 2008 through August 31, 2009.
ELIGIBLE VEHICLES:
All new and unused 2009 model year Mazda vehicles sold to a qualified Daily Rental Fleet Customer in one of the fifty(50) United States or District of Columbia that are ordered through Mazda’s Corporate Fleet Department. This program does not apply to vehicles purchased out of dealer’s stock. MAZDASPEED and MX5-RHT vehicles are excluded from this program.

DOCUMENTATION:
In order to participate in and receive benefits under this 2009 model year Mazda Fleet Program, the Dealer must complete and submit to Mazda the following Mazda Fleet Program forms, as applicable:
- APPLICATION FOR FLEET ACCOUNT ELIGIBILITY (MFP-1):
This form must be submitted to Mazda prior to ordering to qualify the customer as an Eligible Fleet Account. If approved, Mazda will issue a Mazda Fleet Identification Number (F.I.N.) to the customer. This form is not required for a fleet customer who already has a valid Mazda F.I.N. - ACCOUNT ORDER FORM (MFP-2):
This form is used when ordering Mazda fleet vehicles. An ordering F.I.N. and an end-user F.I.N. are required. Be sure to include the account name, Mazda F.I.N., delivery information and vehicle specifications. Fax the form to (949) 727-6474.
